What To Expect
Fit Program Medical Evaluation
At your Fit Program Medical Evaluation, one of our aesthetic nurse providers will review your medical history, labs, set minimum and goal weights, and take Before photos (from your neck to knees - for tracking your progress).
Bloodwork
Below are the required labs we will need to review before starting you on the Fit Program. :
CBC (complete blood count)
CMP (complete metabolic panel)
TSH (thyroid panel)
HgA1C (hemoglobin A1C)
*Labs need to be from less than a year ago
*If you do not have labs, you can still get a medical evaluation, however you will not be able to begin the Fit Program until your labs have been reviewed. Eligibility is not guaranteed.
*You can show us a photo/print-out of your labs or email them to [email protected]
Pre Screening Questions
1. What is your BMI?
2. Do you have Diabetes type 1 OR 2?
3. Do you have a personal or family history of medullary thyroid cancer or MEN syndrome?
4. Are you pregnant, breastfeeding, or trying to become pregnant?
5. Do you have a personal or family history of pancreatitis or other pancreatic disease?
6. Are you under the age of 26 or over the age of 65? *
If you answered yes to any of the questions above, and/or your BMI is below 18 or above 35*, you are not eligible for this program at RESTOR.
*If your BMI is over 30, talk to your Primary Care Provider, weight loss injections could be covered by your insurance.
* If you are under the age of 26, or over the age of 65 you must meet additional criteria to be eligible.

After you complete the pre-screening questions to see if you are eligible above, you can set up your initial Fit Program consultation.
*We have very limited spots available so this is on a first come basis.
*If you do not have labs from the past year, we can do the initial Fit Program consultation but cannot start the program until the necessary lab are complete. To see the necessary bloodwork we need, refer to the "Bloodwork" section above.
